Help working out what's possible

Support coordination

We help you understand your plan, find providers who fit, and build the confidence to make your own choices.

WHAT'S INCLUDED

What you actually get

  • Someone who reads your plan properly and explains it in plain English

  • Help finding providers who actually have capacity right now

  • We make the first calls with you — or for you, if you'd rather

  • Support at your plan review, including what to ask for

  • Someone in your corner when a provider gets it wrong

  • Building your confidence to do more of it yourself over time

What support coordination is for

A plan is a document. Support coordination is the person who helps you turn it into an actual life.

Most people come to us with a plan they don't fully understand, a vague sense that they're not using it well, and no idea who to ring. That's completely normal — the NDIS is genuinely complicated, and nobody hands you a map. Our job is to read it properly, tell you what's in there, and then help you spend it on the things that will make a difference.

The practical part is finding providers. Not a list off a website, but people we've actually dealt with, who have capacity now, and who suit how you like to be supported. Because we're local, we know which services in the Wollondilly are worth your time and which have a six-month waitlist.

The part we care most about is the part where you need us less. Good support coordination should slowly work itself out of a job — teaching you the system rather than standing permanently between you and it. If you're more confident at your next plan review than you were at this one, we've done it right.

Getting started

Four steps, and we do all of them.

1

We read your plan

Properly, line by line, and then tell you what it actually means in words you'd use.

2

We work out what matters

Not what the plan says your goals are. What you'd genuinely like to be different.

3

We find the people

Providers with real capacity, in your area, who suit you. We do the ringing around.

4

We step back

Gradually, as you want us to. The aim is that you need us less each year, not more.
