Navigating the NDIS can feel overwhelming. There are plans to understand, providers to find, budgets to manage, and goals to work toward — all while trying to live your life. That's where WTC's Support Coordination team comes in. We are your guide, your advocate, and your biggest supporter every step of the way.
What is
Support Coordination?
Support Coordination is a funded NDIS support that helps you understand and make the most of your NDIS plan. It is not just about finding services — it is about building your confidence, your independence, and your ability to navigate the NDIS on your own terms.
A Support Coordinator works alongside you to:

Understand what your NDIS plan means and what you are funded for

Find and connect you with the right supports and services

Make sure your providers are working together effectively

Help you prepare for your plan review

Build your capacity to manage your own supports over time

Advocate for you when things get complicated
Support Coordination is different from Support Work. Your support workers help you with day to day tasks and activities. Your Support Coordinator works at a higher level — making sure the whole picture of your support is working the way it should.
At Wollondilly Total Care, we believe Support Coordination should feel like having a knowledgeable friend in your corner — someone who knows the NDIS inside out, understands your local community, and genuinely cares about your outcomes.
We are not a large call centre operation where you speak to a different person every time you call. When you work with WTC, you have a dedicated coordinator who knows your name, knows your story, and knows your goals. That consistency matters — especially when you are navigating something as personal as disability support.
Our coordinators are based right here in the Wollondilly. We know the local services, the local providers, and the local community. That local knowledge means we can connect you with supports that actually fit your life — not just whatever is available on a national database.

Becki D — Support Coordinator
Becki is the heart of WTC's Support Coordination service. Friendly, knowledgeable, and fiercely devoted to her participants, Becki brings both professional expertise and genuine warmth to everything she does. She has a deep understanding of the NDIS framework and a talent for translating complex plan language into clear, actionable steps.
Becki's approach is always person-centred. She takes the time to truly understand what matters to you — your goals, your preferences, your family situation — and builds your support around that. Her participants consistently describe her as someone who makes them feel heard, valued, and genuinely supported.
Step 1
Getting to Know You Your journey with WTC starts with a conversation. We take the time to understand who you are, what your NDIS plan includes, and what you want to achieve. There is no rush and no checklist — just a genuine getting-to-know-you conversation.
Step 2
Understanding Your Plan We go through your NDIS plan with you in plain language. We explain what each support category means, how your funding works, and what you are able to use it for. No jargon, no confusion — just clarity.
Step 3
Building Your Support Network We identify the supports you need and connect you with quality, vetted providers across the Wollondilly and surrounding areas. We coordinate between your providers to make sure everyone is working toward the same goals.
Step 4
Ongoing Support and Check-ins We stay in regular contact to make sure everything is working well. If something isn't right, we act quickly. If your needs change, we adapt. We are always just a phone call away.
Step 5
Plan Review Preparation When your plan review is approaching, we work with you to document your progress, identify any gaps, and prepare a strong case for the funding you need going forward. A good plan review can make a significant difference to your outcomes — and we take it seriously.

Help With?
Support Coordination can assist across many areas of your life, including:
Daily Living
Finding the right support workers, coordinating personal care, household tasks, and community access supports.
Health and Wellbeing
Connecting you with allied health providers such as occupational therapists, physiotherapists, speech pathologists, and psychologists who are the right fit for you.
Housing and Accommodation
Exploring housing options, supported independent living, and specialist disability accommodation if that is relevant to your situation.
Community and Social Connection
Identifying local groups, activities, and programs that align with your interests and help you feel connected to your community.
Employment and Education
Supporting you to explore pathways into employment, training, or further education if those are goals you want to work toward.
Crisis and Complex Support
When things get complicated or a crisis arises, your WTC Support Coordinator is there to help you navigate the situation, coordinate responses, and ensure your safety and wellbeing.
Support Coordination is not automatically included in every NDIS plan. It is typically funded for participants who have more complex support needs or who are newer to the NDIS and need more assistance getting started.
